You’ll be required to travel across the City, to meet and consult with vulnerable people. You’ll be provided with equipment to support agile and flexible working. You’ll be prepared to work out of hours on occasions to attend meetings or conduct visits.
In this role you will assess and manage case work the needs of and risks to people with social care needs including mental health issues, substance misuse issues, physical, sensory, age-related and learning disabilities and their carers You will provide and present reports and maintain up-to-date records consistent with agreed social work recording practice. You will co-ordinate and arrange various community care services and advise and explain financial implications to clients. You will be part of a larger social work service, predominantly based in the office environment however you will be expected to be prepared to visit clients at their home addresses.
You will need to demonstrate a commitment to working in partnership with people and carers to assess needs, identify outcomes and plan support to meet these. Excellent assessment, risk management, organisational and communication skills are essential. You must be qualified in Social Work (or equivalent- please view job specification) and Social Work England registered.
