Job purpose

We are looking for someone to co‐ordinate the efficient and effective delivery of reablement care to service users of Leicester City, through the use of a dedicated IT system.

You will inform Reablement Assistants of any changes to their work programmes on a day‐to‐day basis and ensure the continuity of timing of calls is maintained at all times by using new electronic, rostering and monitoring system.

You must be highly motivated, dynamic and able to respond quickly and professionally under pressure.

You will

Answer all queries relating to the programming of Reablement calls.

Receive and responds to all incoming referrals by setting up Reablement runs which are efficient, effective and personalised by liaising with all key stakeholders.

Ensure every call is allocated and covered and that there are no missed calls by utilising the ERM System.

Provide basic performance data on a daily basis to the management team and key stake holders.

Work alongside all partners as part of a multi-disciplinary approach by ensuring the programing of all calls is in line with individual Reablement personalised plans.

Requirements of post

Previous experience of working within a care setting, which includes the ability to understand the needs of older people including those with a physical disability, a learning difficulty or a mental health diagnosis.

Experience of prioritising work and working under pressure to meet tight deadlines.

Experience of working with computer based applications which includes inputting, extracting and verification of database records.

Previous clerical/administrative experience.

Knowledge of current care standards and legislation in relation to service delivery.

Subject to an Enhanced DBS check.

Additional information

The salary for this post is £19,554 - £21,166 per annum.
