About the job

If you have strong social work values and a clear social care perspective, we can offer you a great opportunity to undertake a specialist role as a Social Worker within our Adult Social Care service, working with young people transitioning into adult services. You must have a good understanding of the Care Act 2014, MCA 2005, MHA, s117’s, Human Rights and Community DOL’s and other relevant legislation.

The Transitions Team works alongside the Learning Disability Service supporting young people who may have long standing health conditions such as learning disabilities, physical disabilities, mental health issues, ADHD, Autism, Asperger’s, sensory impairments as well as other vulnerabilities. A large proportion of our referrals are received from children’s social care teams, specialist schools, colleges families, self-referrals and health and other professionals. We work closely with our education partners and attend reviews for those young people with an EHCP from as early as 16 years of age and look at delivering their aspirations for further education, employment, and greater independence around management of everyday living skills (accommodation and finances when identified).

To remain person centred in your daily practice ensuring that the young person/adult is empowered through positive strength-base practice. The team’s purpose is to focus on identifying support needs ready for adulthood in ensuring a seamless transition from children’s services. To meet the needs of a diverse multicultural community.

You’ll be part of a larger social work service, predominantly based in the office environment however you’ll be expected to be prepared to visit clients at their home addresses.

Your role

Your role will involve working in a busy team to meet the diverse needs of young people in the city of Leicester and ensure that a good transitions journey is undertaken into adulthood. 

As an experienced Social Worker, you’ll assess and manage a case load of complex cases which will include people with mental health issues, substance misuse issues, physical, sensory, age-related issues and their carers.

You’ll provide and present reports and maintain up-to-date records consistent with agreed social work recording practice. You’ll undertake section 42 enquiries, VARM’s, court of protection, attend and actioning multi-disciplinary case work, attend CTR’s, CPA’s and EHCP Reviews. 

Coaching and developing is key in our field and you’ll offer support to our student social workers, care management officers and level one social workers.

Due to the nature of the service you’ll be expected to undertake an active part in a busy duty system.


What you'll need

You’ll be qualified in Social Work and registered with Social Work England, and you’ll have the ability to work with people, at their own pace to create innovative and appropriate support packages in order to maximise independence whilst minimising risk.

You’ll demonstrate strong interpersonal, communication and organisational skills with the ability to analyse and interpret information, monitoring work and evaluating practice to identify any unmet needs.

With a commitment to working in partnership you’ll show the ability to work within a multi-disciplinary team particularly with specialist staff from other agencies such as Health, Probation, and Housing.

Using your experience of working with young people and adults with a range of social care, educational, health and housing needs, you’ll confidently work with the young person/adults and carers to assess needs, plan support to meet these needs, and identify strength-based outcomes.
